Required documents
- A packing list, notarized by a notary public.
- An affidavit signed by the applicant certifying that he or she has resided in the United States for a period exceeding three years, intends to leave the United States and settle in Lebanon for good, and that the items transferred were previously owned and used. The affidavit must be notarized by a notary public.
- Proof of three-year residence in the United States (such as Green Card, US passport, or visa).
- Lebanese ID (original or notarized copy).
- A full address in Lebanon and a phone number in Lebanon.
- A self-addressed stamped return envelope.
Fee
- $180 — money order / cashier check payable to the Consulate General of Lebanon in Los Angeles
