Required documents
- The original American death certificate, or a certified copy.
- A burial transit permit.
- A letter of non-contagious disease issued by an authorized health institution.
- A letter of embalming.
- A letter of sealed casket.
- The original passport of the deceased (to be cancelled).
- The Lebanese extract of civil status (Ikhraj Qayd) of the deceased.
- The death registration form, filled and signed by a member of the family, also emailed to Info@lebanonconsulatela.org with the requested documents attached.
- A self-addressed stamped return envelope.
Fees (money order payable to the Consulate General of Lebanon, Los Angeles)
- $50 — transfer of body
- $60 — registration of death
