Required documents
- The completed death registration application form.
- The official American death certificate.
- The Extract of Civil Status of the deceased, his/her Family Extract, or his/her Lebanese identification card.
- The Lebanese passport belonging to the deceased. The passport will be cancelled and mailed back.
- A return self-addressed stamped envelope.
Optional
- $60 — affidavit for death certificates, provided by the Consulate General upon request
