Registration of Birth

Before starting any request, verify that your state of residence falls under this Consulate’s jurisdiction.

The completed birth registration application form.
The official American birth certificate.
The parents’ Family Extract of the Civil Register.
The parents’ marriage must be registered prior to the registration of their children. If the marriage has not yet been registered, it is possible to register the marriage and the birth at the same time.
A notarized copy of the birth certificate for non-Lebanese mothers. An official translation to Arabic is required if the certificate is neither in English nor in French.
A return self-addressed stamped envelope.

$60 — affidavit for birth certificates, provided by the Consulate General upon request

Fill out the online form and send all listed documents to the Consulate’s mailing address.
