Required documents for applying for a new extract of the civil register (Ikhraj Qayd):
- A valid Lebanese document: Lebanese national identity card, passport, or old Ikhraj Qayd.
- A copy of an American document: valid Green Card or American passport.
- Two recent color photos (4.3 × 3.5 cm, not drawn from another photo) showing full details of the face on a white background.
- A completed application form (available from the Consulate or the Forms & Downloads page).

Individuals unable to appear in person at the Consulate General can fill out the application, have it notarized before a notary public, and mail it to the office of the Consulate General.
