Registration of Death

Before starting any request, verify that your state of residence falls under this Consulate’s jurisdiction.

The completed death registration application form.
The official American death certificate.
The Extract of Civil Status of the deceased, his/her Family Extract, or his/her Lebanese identification card.
The Lebanese passport belonging to the deceased. The passport will be cancelled and mailed back.
A return self-addressed stamped envelope.

Optional

$60 — affidavit for death certificates, provided by the Consulate General upon request

Fill out the online form and send all listed documents to the Consulate’s mailing address.
