Power of Attorney

In person

Select the type of POA you would like to do from the samples list below (نماذج الوكالات — the list is not exclusive). Required documents:

• Original valid Lebanese ID of applicants (إخراج قيد أو تذكرة هوية أو جواز سفر).
• A copy of the recipient’s ID (إخراج قيد أو تذكرة هوية أو جواز سفر).
• Original certificate of good mental health issued by a medical doctor, for applicants over 70.
• A copy of the applicant’s driver’s license and phone number.

Additional documents for an in-person Special POA:

• Real estate: a copy of the original deed, original affidavit of ownership, or a registry certificate (افادة عقارية أو سند ملكية).
• Cars: a copy of the car registration record.
• Bank accounts: the bank address and account number.
• Divorces: a copy of the marriage certificate or family civil record.
• Maids / housekeepers: a copy of the housekeeper’s passport.

By mail — General POA (legalization)

The Consulate legalizes powers of attorney executed before a notary public. Select the required POA from the samples below, then submit:

• The filled-out General POA form, notarized by a Notary Public and then authenticated by the Secretary of State (follow the notarization and authentication instructions included in the form).
• A photocopy of the filled-out and authenticated General POA form.
• Applicant’s ID and a copy of the recipient’s ID.
• Applicant’s phone number and email address.
• Copies of the witnesses’ IDs (passport or driver’s license).
• Applicants over 65 must include a certificate of good mental health from their medical doctor. For American citizens, an original valid US passport is required.
• Money order, credit card, or cashier check payable to the Consulate General of Lebanon in Los Angeles — $100 per signature (add $40 for true copies).
• Stamped, self-addressed envelope (USPS or FedEx only).

By mail — Special POA (legalization)

• Fill out the corresponding Special POA (follow the notarization and Secretary of State authentication instructions included in the form).
• Copies of the witnesses’, applicant’s and recipient’s IDs.
• Applicants over 70 must include a certificate of good mental health from their physician.
• For the sale of property: include a copy of the deed of trust; the number and location of the property must be clearly mentioned in the POA.
• A money order / cashier check payable to the Consulate of Lebanon — $50 per signature.
• Stamped, self-addressed envelope (USPS or FedEx only).
